MHT225 — Community Integration and Inclusion
Building upon an understanding of the social systems model, this course examines the various approaches to case management in a range of settings. Students will learn to perform a thorough psychosocial assessment to inform a collaborative, person-centered and outcome-focused plan of care and use of systemic and natural supports to facilitate an individual’s progress toward their goals.
Prerequisites: MHT201, MHT130
