Focuses on process of collecting and organizing client information from initial intake through discharge planning. Includes hands on experience administering, scoring, and interpreting screening and assessment tools commonly used in human services, with applications in areas such as mental health, substance use, and daily living. Teaches compiling client information to formulate treatment and service plans, coordinating providers and resources, and documenting case management activities. Emphasis is placed on ethical practice, confidentiality, effective communication, and accurate documentation, including progress notes, treatment planning, levels of care and discharge planning.
